Online Forms

We have a variety of forms our clients can use to apply for reduced co-pay, apply to be on the board, or file a complaint. Find the relevant form and fill them out below.

Reduced Copayments

If your income falls below the federal poverty line, you're eligible for our reduced copayments program. When approved, take 50% off any paid service through us.

Board of Directors Application

Our Board holds the Administrative Team accountable and helps shape the future of our agency. If you'd like to participate we encourage you to submit an application.

Client Grievance

We take any issues our clients face doing business with EDI very seriously. If you ever encounter an issue, file a grievance and our administrative team will work with you to come to a resolution.

Refusal Request

If you run into any issues with a particular Team Member, you are encouraged to file a refusal request. If approved, they will no longer be sent on any calls involving you.

If you're looking for clients in need of Caregivers our Case Managers are here to help. Apply for referrals and if approved we'll send you clients.
